ATHLETICS FORMS

Each student athlete needs a current MSHSAA PRE-PARTICIPATION PHYSICAL-MEDICAL ELIGIBILITY and MSHSAA PRE-PARTICIPATION DOCUMENTATION – ANNUAL REQUIREMENTS Form on file in the Athletic Department each year she participates in a high school sport. Athletes are not allowed to try out for their sport until they have a current Athletic Physical and Annual Required Pre-Participation Form is on file.

MSHSAA requires that all athletes and parents read the MSHSAA Materials on Concussions, which includes information on the definition of a concussion, symptoms of a concussion, what to do if you have a concussion, and how to prevent a concussion. Both athlete and parent must sign the MSHSAA Annual Pre-Participation form to indicate they have received and read the concussion information.

Physicals for those participating in fall sports need to be turned in prior to tryouts. Freshmen and those without a physical last year need the medical eligibility form. Those who make teams will also need the pre-participation document returned.

ATHLETICS PARTICIPATION FEE

After being selected for a Nerinx athletic team, each athlete will be billed a $150 participation fee through Smart Tuition. This is a one-time-per-year fee per fee, regardless of the number of sports in which the student participates. The Athletics Participation Fee will cover the cost of uniforms, facility rentals, tournament fees, some transportation costs, equipment, and a possible out-of-town trip. All uniforms will be collected at the end of the season. If the uniform is not returned, an additional fee will be assessed.
